Players take on roles in a psychiatric hospital where the focus is interpersonal horror and the psychology of fear rather than supernatural monsters. Gameplay emphasizes investigation, roleplaying, and character interaction as staff, patients, and visitors reveal personal traumas, ethical conflicts, and escalating tensions. The scenario presents a scripted sequence of scenes, key locations, and a cast of detailed non-player characters to portray; referees use these materials to stage murders, suicides, and other incidents that test characters’ sanity and professional boundaries. Mechanics center on managing fear, making difficult treatment or moral choices, and uncovering how power, obsession, and repression distort perception and behavior.
