Players portray characters who once fled an old sanatorium now haunted by vengeful forces and must try to survive its return. The game unfolds as a cooperative, narrative experience in which players create linked scenes across a board that tracks time in quarter-hour increments and holds several card decks. Play divides into two main phases: an Exploration phase of ten rounds where players build scenes by playing cards that specify hour, character, location and events; and a Regression phase of ten rounds where players attempt to recall the key elements from those scenes. Failures to remember trigger penalty cards that threaten characters’ survival, and the group wins if any characters remain alive at the end (with a score reflecting survival), or loses if all characters die.
