An abandoned psychiatric facility becomes the setting for asymmetric cat-and-mouse play. One side takes on twisted killers — a perfection-obsessed Doctor and a Nightmare-possessed little girl — while the other controls survivors trying to escape the asylum's corridors and rooms. Gameplay adds fear and stress management as mechanical elements through encounter cards and new tokens, and includes dedicated maps, killer boards, miniatures, and skill cards. Players move, hide, and resolve skills and encounters as tension builds; killers hunt and use unique abilities while survivors cope with stress effects and attempt objectives to flee.
