The Book of Plagues

This rules supplement adds around 30 real-world diseases to fantasy roleplaying campaigns, from potentially lethal illnesses such as bubonic plague to milder conditions such as swimmer’s itch. It includes classic ailments such as dysentery and leprosy, with guidance for incorporating them into adventures. Each entry provides contraction chances, transmission circumstances, incubation periods, and symptoms compatible with D&D 5E, 2024 rules, and both editions of Pathfinder. Diseases can consume healing resources, drive characters from dangerous locations, add premodern realism, or create threats through creature vectors such as mosquitoes and ticks. Random affliction tables offer additional options.
