You can play any number of Fight cards during a fight action (after rolling) and any number of Defend cards when suffering damage; Travel and Heal cards are free actions and may be played any number of times on your turn, while Stronghold cards are resolved immediately when drawn (pages 7, 11).

Enemies deal damage after you resolve your hits: each ghoul and abomination on your space deals 1 damage (so damage is per enemy present, not per hit rolled); shield results reduce that damage and the Lich King adds +1 damage for fights in his region (pages 5, 7).

No — cards contributed to a quest are not discarded and remain in your hand, and each hero on the quest space can contribute one card per quest action (pages 6, 10).

Resolving 'The Scourge Rises' advances the Scourge marker, moves the Lich King to the region, adds ghouls up to three on that space, and places an abomination; during the regular Spawn Ghouls step, attempting to place a fourth ghoul causes an overrun instead (advance despair once and place an abomination) (pages 8, 10).
