Players enter the surreal realms of the Dream Cycle and confront dream-haunted threats that alter the battlefield each scenario. The expansion adds a new die type and modular tiles that shift during play, introducing unpredictable terrain and effects. Players control investigators with unique abilities, manage sanity and relics, and face new monsters and an additional Elder One across six scenarios. Gameplay emphasizes cooperative tactics, crisis resolution, and long-term campaign consequences as investigators battle to complete episode objectives while resisting escalating Insanities.
