Players each lead a three-character Mafia crew — killer, dealer and vamp — and vie to control suburbs or eliminate rival gangs. On a turn they move characters to steal money, rob or racket shops, deal drugs, vamp opposing characters, bribe or confront cops, and hire reinforcements to replace losses. Combat is resolved with dice to determine initiative and a unique dart-shooting duel: attackers and defenders exchange dart shots based on dice differences until targets are eliminated (face-down means killed, face-up wounded). A player is eliminated when all three crew members are killed; an optional fifth player controls the police with arrest and testimony objectives.
