Players lead rival noble houses tasked with saving a plague-ravaged empire by reclaiming ruined cities and caring for survivors. Play proceeds over six rounds; each round begins with a revealed plague card that dictates how the disease spreads. On a turn, players use an action-selection system on their player board to move containment teams, quarantine and cure infected people, rebuild infrastructure, assign workers, harvest resources, and develop technologies. Houses grow influence to enable more actions, generate resources, and house more survivors. The game ends after six rounds and players score Renown to determine who best restores the empire.
