Players are plague doctors in 1347, treating the infected while seeking fame, prestige, and wealth. They secretly invest influence tokens across six areas to compete for actions such as recruiting commoners, managing gravediggers, building developments, draining infected swamps, and establishing plague hospitals. Across three periods, players reveal their investments, resolve area actions, and maintain their healed commoners. These commoners provide additional influence and abilities, while coins, subjects, developments, and influence contribute to wealth points at the end of the game.
