Nearly two thousand years after humans overthrew the dragon-god emperor Saatman, a cult restores him to life. Declaring war on the Klavek Kingdom, he commands the destruction of all humankind, leaving the party to prevent genocide and turn his former allies against him. The adventure includes the city of Gustavin, where feuding houses control trade and dragons are worshiped as gods, along with reputation and infamy rules, maps, hidden lore about the Klavek Kingdom, and new threats and magic. These include the spell Genocide, the stitched-together Serpens, the magic-storing Scrollblade, and the powerful but costly Radiant Elixir.
