Players represent communities across medieval Europe and North Africa as the Black Death spreads through the regions. Each round, players recruit or use medieval professions—peasants, bakers, monks, knights, and others—to grow population, protect against disease, or inadvertently spread the plague. Cards and modules introduce events, special roles, and region effects that alter who survives. The game combines base play with multiple expansions and optional modules that add additional professions, bonus cards, and upgrade tiles for greater variety and replayability. Play ends when the plague recedes and the surviving population determines the winner.
