Players assume the roles of prominent Messina families fleeing the city during the arrival of the plague. They send ships, rescue citizens, and relocate them to their countryside estates where rescued inhabitants can work and contribute to the household. Back in town, players dispatch efforts to combat the spreading infection and influence districts; actions balance saving people, managing estate resources, and preparing to return and assert control over urban areas once the epidemic subsides.

If I rescue a citizen from a plagued hex and then remove the plague on the same turn, can I place that citizen in my sector instead of quarantine?
No — a citizen rescued from a hex that has a plague cube must be placed in quarantine immediately, even if you remove the plague later that same turn (p. 8).
Can I use citizens who are in quarantine or who are working in a workshop to repopulate a neighborhood?
Citizens in quarantine cannot be used to repopulate Messina (they stay in quarantine for two rounds) (p. 13), but a citizen that is in a workshop can be sent to repopulate a neighborhood (p. 12–13).
When do I gain rat tokens, and what do they do at the end of the game?
You gain rats if plague cubes remain after you spend fire tokens on a hex and also whenever a plague cube is added to a neighborhood you repopulated (p. 8, p. 13). At game end each rat moves you back on the popularity register and then you lose points according to the rat-penalty table (p. 17).
If I upgrade an overseer, does the upgraded overseer’s extra activation work immediately when it next advances — including if I upgrade-and-advance in the same effect?
An upgraded overseer provides extra activations when it advances; if an effect both upgrades and then advances that overseer (for example a church-register space), the upgraded ability applies during that advance (p. 11, p. 14).
If I use a major fire token on a boat, can I also remove a plague cube from an adjacent hex?
No — the plague cube on a boat is not considered adjacent to any hex, dock, or other boat, so a major fire token used on a boat cannot reach an additional adjacent cube (though you may use a major fire token as a regular fire token) (p. 9).
If I advance on the city register, can I take the action shown on a hex even if a standing lieutenant blocks that hex?
Yes — when you advance to the indicated city-register space you may take the action depicted on any hex in Messina even if that hex is blocked by a standing figure, but this permission does not apply to docks (p. 14).
